Do we have here a type of ‘double causality’? My destiny is determined, but how I actualize that destiny is my own? Does this mean I have somewhat ‘potential’ lives that exist at least as potentials, until I choose which I make real? What you call ‘various eternal states of well-being’?

I guess I'm suggesting that WHAT we are is wholly determined, naturally per logoi, and so is WHO we are - our identities mutually constituted.

HOW we manifest Christ, individually per tropoi, is synergistically caused as we cooperate with the Spirit, to a great extent co-creatively self-determining our peculiar way of expressing the logoi.

"In a way both creative and unexpected, the Holy Spirit generates diversity, for in every age he causes new and varied charisms to blossom. Then he brings about unity: he joins together, gathers and restores harmony".

~ Francis on Pentecost 2017

By states of well being I refer mostly to our perpetual growth (as images) in likeness to & intimacy with the Christ. So, in my view, we're choosing among "potential" spiritual gifts, charisms & secondary beatitudes, thereby peculiarly expressing the logoi, individually manifesting the Christ.
